How is Perineal Reconstruction performed?

Tissue flap surgery is the most common type of perineal reconstruction. This surgery involves taking healthy tissue from another part of the body and using it to repair the defect. The type of flap used will depend on the location and size of the defect.

The types of flaps usually used during perineal reconstruction are:

Gracilis myocutaneous flaps

Gracilis myocutaneous flaps are the most commonly used flaps in perineal reconstruction. The gracilis muscle is a groin muscle. It begins at the top of the pubic bone and runs down to the upper half of the shinbone (tibia). The gracilis muscle, along with skin, subcutaneous tissue, and fat with a plentiful blood supply, is used in the gracilis myocutaneous flap.

Abdominal flaps

Abdominal flaps are used when the perineal defect is large and complex. This flap uses skin, subcutaneous tissue, and muscle from the abdomen. There are two types of abdominal flaps: DIEP flap (deep inferior epigastric perforator flap), and TRAM flap (transverse rectus abdominis myocutaneous flap).

Thigh flaps

Thigh flaps use skin, tissue, and muscle from the thigh. Thigh flaps are versatile with many excellent attributes and can be used to cover most defects. It is also relatively easy to harvest.

Groin flaps

Groin flaps are vascularized flaps. This means they involve harvesting a flap of skin and tissue along with the attached arteries and veins from the groin area. It is a safe and reliable way to cover soft-tissue defects.

Gluteal flaps

Gluteal flaps are another type of vascularized flap. They involve harvesting a flap of skin and fat along with the attached arteries and veins from the gluteal region (buttocks). Gluteal flaps can be used to cover large defects.

Rectus femoris flaps

The rectus femoris muscle is a long muscle that extends from the hip to the knee. The rectus femoris flap is a myocutaneous flap, which means it uses skin, subcutaneous tissue, and muscle. This flap is usually used to cover large defects.

Perforator flaps

Perforator flaps are used in reconstructive surgery because they allow for less invasive and more complex reconstruction by preserving important blood vessels and muscles.
